Explorer (Details Option)
Click on a blank area in the right pane. Hit Ctrl and + at the same time. That's the + key on the numeric keypad. This usually resets columns and fixes this. Selecting Details though just gives a blank area, with not even the heading bar segmented into Name Size File Type, Last modified etc.

Icons change in Excel
I'd like them to be "Name", "Type", "Date Modified" and "Size". The Documents folder is correct, but I have to change each other folder manually and that takes some time as I can only remove one column at a time.
